Important Benefits Information As You Leave the MBL:

Health Insurance Coverage (Medical and Dental)
If you are enrolled in the MBL’s Medical or Dental plans your coverage will end either mid-month or at the end of the month, depending on the timing of your last payroll deduction for your health insurance. You may be eligible to continue this coverage by electing COBRA. Detailed information about COBRA will be sent to your home address within two weeks after your last day of employment, or will be delivered in-hand during the Exit Process by Human Resources.

LTD/Life/AD&D Coverage
Long Term Disability, Life and AD&D (accidental death and dismemberment) Insurance coverage through SunLife Financial Assurance Company ends on your last day of employment. Your Life Insurance can be converted to an individual policy; applications for Conversion must be received by SunLife within 31 days of your termination date. LTD and AD&D do not have the same conversion privileges. Please contact Human Resources for a policy conversion application.

Vacation and Sick Benefits
Any vacation hours that you have earned and accrued but not yet used will be paid to you in one lump sum with your last paycheck. Unused sick time is not payable upon separation of employment.

Group Retirement Annuity/Supplemental Retirement Annuity (GRA/GSRA) or other TIAA-CREF Contracts:
Participants of these contract annuities may call TIAA-CREF to learn about options upon termination. TIAA-CREF gives you round-the-clock access to your TIAA-CREF account via the internet at, or you can call the Telephone Counseling Center to speak with a consultant at 1-800-842-2776. The MBL is not involved with this account after your last day of employment; any withdrawals or movements to your accounts are managed by you directly with TIAA-CREF.

HR will only confirm dates of employment and job titles. Other information will not be provided unless required by law.

Healthcare Flexible Spending Account
The last payroll deduction to these accounts will occur with your last paycheck. However, you have the right to continue to contribute to your FSA with after-tax contribution under the provisions of COBRA.  If you have a balance in your account you may continue to submit claim forms with the appropriate receipts for expenses to HRC. You have 90 days from your termination date to submit claims. Unless a COBRA election is made, only claims incurred through your last day of employment may be reimbursed.

Dependent Care Flexible Spending Account
The last payroll deduction to these accounts will occur with your last paycheck.    If you have a balance in your account you may continue to submit claims for services incurred through the end of the plan year to HRC. You have 90 days from the end of the plan year to submit claims.
