Vendors and Contractors Visiting the MBL

Vendor and Contractor
Visitation Form
The MBL is committed to establishing a safe campus environment that will enable the resumption of our most fundamental research and educational activities. In coordination with the University of Chicago, we are developing plans for a safe, coordinated, and phased return to campus operations. These phased plans and precautions focus on key components of the MBL based on Commonwealth of Massachusetts requirements under the Four-Phase Reopening Plan.

As we begin to reopen the MBL we expect all members of the MBL community and any visitors to the campus to adopt new precautions as a requirement for returning to campus. These precaution measures will help us to achieve our goal of providing a safe environment for our entire campus community. Please remember that we are still in a dynamic COVID-19 environment.

All vendors and contractors visiting the MBL campus to conduct repair or service work on equipment or facilities must complete the Vendor and Contractor Visitation Form prior to arriving on campus and must also read and abide by MBL’s Health Policies and Protocols below.

Thank you for adapting to these challenges, and for your dedication to safeguarding our MBL community.
