Registration Forms

Every person who comes to the MBL to conduct research, participate in an education program, attend a conference, or rent library space is expected to complete a registration form. This does NOT apply to course students.

We implemented a new registration system on January 1, 2021. If you completed a registration since then you are considered a “Returning Account Holder”. If you have not completed a registration since then you should use the “First Time Registration” link.

COVID-19 Vaccination Registry and Safety Acknowledgement and Attestation

All registrants are required to complete the vaccination registry and COVID-19 safety acknowledgement and attestation, which are part of the registration form.  Students complete this same information as part of their decision reply form. The registry and attestation are a pre-requisite for obtaining an MBL identification and door access card (MBL Card).

COVID-19 Vaccination Registry and Safety Acknowledgement and Attestation

Two weeks prior to your arrival date, approved registrants will receive an email from MBL containing links and instructions for completing the MBL’s vaccination registry and COVID-19 safety acknowledgment and attestation. The registry and attestation are a pre-requisite for obtaining an MBL identification and door access card (MBL Card) and must be completed at least 48 hours prior to arrival on campus.
